The School Outreach & Support (SOS) Program 

The School Outreach & Support (SOS) Program at YES Community Counseling Center offers free support to students and families who are facing - or at risk of - substance misuse and mental health challenges. Funded by a grant awarded by Nassau County, SOS partners with school districts including Island Trees, Levittown, Wantagh, and Farmingdale.

We provide seamless access to care during the school day, after school hours, and evenings - whether at school or at our counseling center. Our Collaborative Care Coordinators meet with students wherever support is needed, ensuring ongoing, free care and assistance.

Services Provided

Individual and group support services are provided on a short-term basis when emotional or environmental issues are impacting upon academic or social performance within the school building. When working with individual students and families, the role of The School Social Worker/Intern is to identify issues that exist, assess needs, and provide an appropriate referral to the student and family.

Supervision

Supervised on a weekly individual and group basis, the prevention counselors placed in your school have the opportunity to case conference and receive feedback for intervention. Additionally, the agency’s supervision team ensures that workers are properly trained and updated on licensure, certification, and current trends, particularly as it relates to chemical dependency.
